One-Time Payment Authorization Terms and Conditions

You authorize Us to charge to the Payment Method You provided today, if any, or another previously stored Payment Method You designated the payment amount, if any, You specified today on the payment date (today or a later date) You also specified. Your Payment Method could be charged as early as today or on the date You have specified for payment. 

A late fee may be applied to Your Deltapath account if You schedule a payment to occur after Your due date. If a charge to Your Payment Method is declined including for insufficient funds, We may re-submit the charge up to the number of times permitted by network rules, and We also reserve the right to undertake further collection action, including imposing costs and fees to the extent permitted by law. You release Deltapath from any and all claims arising from Your use of this service. 

You may revoke Your payment authorization by calling the customer care number provided with Your bill, which revocation will take effect when Deltapath has had a reasonable opportunity to act on it.

AUTOMATIC AND FINAL PAYMENT AUTHORIZATIONS TERMS AND CONDITIONS

Storage of Payment Method

If You provided Your credit or debit card (“Payment Method”) information to Us today (including by scanning a voided check), You authorize Us to store the Payment Method information. The words “Deltapath,” “We,” “Us” and “Our” mean Deltapath, Inc. and its affiliated companies, and the words “You” and “Your” mean the person providing Us with the Payment Method information and any other person who is authorized to make purchases from Us to be charged to the Payment Method.

You certify that You are the owner of the Payment Method or have authorization to use the Payment Method information You provided, and are authorized to make charges to and permit Us to store the Payment Method You provided. You agree to keep Your Payment Method information up to date, and You acknowledge that Deltapath may receive updated Payment Method information from Your financial institution. If You have previously provided Us with Payment Method information, We may replace that with, or instead store, any Payment Method information You provided today.

Payment Authorizations 

You authorize Us to automatically charge to the Payment Method You provided today, if any, or another previously stored Payment Method You designated the amount of Your monthly bill due each month on the date indicated on Your monthly bill, including any late fees. In addition, You authorize Us to charge to this Payment Method any amounts outstanding if Your Deltapath service is terminated, including any applicable cancellation fees and equipment non-return fees. This termination payment authorization does not cancel, and is in addition to, any other payment authorizations You previously provided for charges assessed if Your service is terminated or may provide in the future for other Payment Methods.

If a charge to Your Payment Method is declined including for insufficient funds, We may re-submit the charge up to the number of times permitted by network rules, and We also reserve the right to undertake further collection action, including imposing costs and fees to the extent permitted by law.

Revocation

Your Automatic and Final Payment Authorization (“Authorization”) and the Deltapath automatic payment (“AutoPay”) service will remain in effect until revoked by You, Your financial institution, or Deltapath. You may revoke Your Authorization by logging into your online account or by calling the customer care number provided with Your bill, which revocation will take effect when Deltapath has had a reasonable opportunity to act on it. If You revoke Your Authorization, or Deltapath or Your financial institution cannot process Your automatic payment, You remain responsible for paying Your monthly payments and any amounts due upon termination of service, including by making monthly payments by check or other payment method on or before the due date on each bill. AutoPay cancellation or unsuccessful payment may cause an interruption of Your service and additional reactivation fees. Promotional discounts or incentives that require AutoPay will be removed if AutoPay is revoked or otherwise terminated. You release Deltapath from any and all claims arising from Your use of AutoPay.

It usually takes one to two billing cycles for AutoPay to start, and please continue to pay as usual until Your monthly bill states that the bill will be paid by AutoPay.
